BIRTH REGISTRATION OF NEWLY BORN CHILDREN
Under the Pakistan Citizenship Act of 1952 all Pakistani Citizens are required by law to register their children who are born abroad with the nearest Pakistani Embassy / High Commission / Consulate General [As given in the last page of Pakistani Passports]. This should done within six (6) months of his/her birth. The requisite form (Form S) given below can be downloaded from this page.
Following are required to be attached with application Form 'S':-
Child's Birth Certificate in original alongwith a copy duly attested by JP
Copy of Father's Pakistani Passports (First 4 pages) & NICOP / CNIC duly attested by JP
Copy of Mother's Pakistani Passports (First 4 pages) & NICOP / CNIC duly attested by JP
Copy of Marriage Certificate / Nikah Nama in case the mother is holding passport in maiden name
Three recent 2x2 size photographs in matte finish of child with name written at the back
In case any one of the parent is not a Pakistani than letter of consent duly attested by JP from him/her will be required or
In case mother is holding Machine Readable Passport and she desires to endorse the child on Fathers Passport than a letter of consent from Mother duly attested by JP will be required
Self Addressed envelope (registered post or prepaid courier)
PLEASE NOTE THAT ENDORSEMENT OF CHILD IS NOT DONE ON MACHINE READABLE PASSPORTS BY THIS HIGH COMMISSION OR CONSULATE. IN CASE THE CHILD HAS TO TRAVEL IT IS ADVISED TO KINDLY APPLY FOR A FRESH PASSPORT FOR HIM / HER.
PLEASE NOTE THAT CHILDREN WHO HAVE NOT BEEN REGISTERED WITH THE HIGH COMMISSION / CONSULATE GENERAL WITHIN THE STIPULATED TIME HAVE THE OPTION TO APPLY FOR A ONE YEAR RESTRICTED VALIDITY PAKISTANI PASSPORT. THIS VALIDITY WOULD BE EXTENDED TO FULL FIVE YEARS ON PRODUCTION OF NICOP CARD. THIS FACILITY WOULD BE SUBJECT TO SUBMISSION OF FOLLOWING STATUTORY DECLARATION
BY THE PARENTS:
" I / WE MAKE THE FOLLOWING DECLARATION UNDER THE COMMONWEALTH OF AUSTRALIA STATUTORY DECLARATIONS ACT 1959 : MY DAUGHTER / SON [NAME] WAS BORN AT [PLACE OF BIRTH] ON [DATE OF BIRTH]. HE/SHE HAS NOT BEEN REGISTERED WITH PAKISTAN HIGH COMMISSION, CANBERRA / CONSULATE GENERAL, SYDNEY NOR WITH ANY OTHER PAKISTANI AUTHORITIES IN PAKISTAN AND ABROAD. IT IS ALSO DECLARED THAT HE/SHE HAS NOT YET BEEN ISSUED ANY PAKISTANI TRAVEL DOCUMENT".
Please fill in the form in BLACK INK and clearly print or type only in CAPITAL letters. A block may be left empty wherever a word ends and a new word begins in the same line.
Avoid any overwriting and other mistakes while filling the form. Please make sure the form is filled as neatly as possible. If there is any mistake is the form, please fill in a new form.
NIC/CNIC number (Field 5 to 14) All numbers must be written without dashed, leave unused spaces BLANK.
Present Address in the form indicates Address in Australia.
Permanent Address in the form indicates Address in Pakistan.
Applicant must put left thumb impression using black thumb pad (with clear impression) and signature on the form.
Photo, Thumb or signature must not exceed allotted space otherwise they will be truncated.
NICOP form must be attested by CNIC / NICOP holder. Parents, brother and sister can not attest the form.
Field-33 is Mandatory. Applicant must fill this column and provide CNIC/NICOP number of his/her Blood Relative. Blood Relative covers following relations (Real Father/Mother, Real Brother/Sister, Real Son/Daughter, Grand Father/Grand Mother)
Field-34 is Mandatory for Muslims
Two passport size colored photographs taken within past three months, with clear face and shoulders; one pasted at the specified place and other may attach at the top right corner of the form.

Supporting Documents.
Photocopy of National Identity Card (NIC) / CNIC.
Photocopies of valid Pakistani Passport with valid visa page or valid Australian Passport.
For married women holding CNIC in maiden name, photocopy of husband’s NIC & marriage certificate.
Photocopies of parents National Identity Cards for those who do not have their own NIC.
NICOP for Children under 18 years.
Completely filled in NICOP form.
Photocopy of RG (II) form or CRC issued by NADRA OR Birth registration certificate issued by the High Commission/ Consulate/Consulate General of Pakistan for children born to Pakistani parents abroad). All copies duly attested by Justice of Peace (JP).
Photocopy of Birth Certificate duly attested by Justice of Peace
Photocopies of valid Pakistani passport with valid visa or Australian Passport.
Photocopies of Parents National Identity Cards.
Child’s left hand thumb impression is mandatory to be fixed in the “Applicant’s Left Thumb Impression box” in field 28.
Parents may sign in case of inability on part of children.
FEES
Bank Draft/Money Order in the name of “Pakistan High Commission, Canberra” for A$ 40 per card. Pl note that in case you desire the card to be delivered directly to your residence, then the charge is A$ 50 per card. For more than one card, a consolidated draft can be prepared for one family.
FOR MODIFICATION/CHANGER IN ALREADY ISSUED NICOP: Kindly highlight the field (s) which you want to modify (Original card is required to be surrendered).
Note:Applicants sending their documents through mail should invariably enclose a pre-paid registered envelope.
